Driver Health Declaration Template
A driver health declaration for confirming a driver is medically fit to drive. It asks about vision, blackouts and fits, medication and treatment, other medical conditions, cardiovascular history and sleep disorders, so any factor affecting safe driving is disclosed and can be managed.
Managing driving for work includes making sure drivers are fit to drive, in line with DVLA medical standards. This declaration gives you a documented confirmation and supports the operational control requirements of ISO 45001.
Built for Driving for Work and ISO 45001
The declaration supports the operational control requirements of ISO 45001 (clause 8.1):
- Vision and general fitness to drive confirmed
- Blackouts, fits and epilepsy declared
- Medication, treatment and other medical conditions captured
- Cardiovascular history and sleep disorders covered, with a signed declaration
